Identity Band Information

Identity bands are used to identify hospital inpatients.

When you are in hospital, it is essential that you are given and wear an identity band with accurate details about you on it. This ensures that staff can identify you correctly and give you the right care.

How You Can Help

You can help ensure that the identity bands are correct by checking that the following basic information is on your identity band:

  • Your first name
  • Your last name
  • Your date of birth


If the information on your identity band is not accurate, please tell a nurse or doctor and ask for an identity band with the correct information on it.

Please do not remove your identification band.

This identification band is needed to ensure that all medications, treatments, and tests are administered correctly.


If you have an allergy, the nurse must make sure that your identity band is red.
