Patient Flow Department
Description of Service
The patient flow staff are a nursing team supporting your admission (either as a day case or inpatient) and discharge from the Mater hospital.
Your full co-operation is greatly appreciated in this matter.
In arranging your discharge your doctors and nurses may decide that your care will be provided in your own environment (home), with the support of a variety of services in the community. This may be in the form of nurse visits, anti-biotic treatment, dressings, physiotherapy and / or blood tests.
Also, your care could be continued in other adjoining facilities under the Mater hospital.
The main responsibilities include:
- Supporting patient placement from the Emergency Department, Out Patient Department;
- Scheduling admissions from waiting lists and transfers from other hospitals;
- Confirming admission dates and communicating this to the patient;
- Promoting day care treatment instead of inpatient care;
- Supporting your discharge from hospital to home by 11am;
- Fully engaging with each patients journey promoting the shortest possible stay in the hospital by working closely with your medical, nursing, physiotherapist, social worker, occupational therapist, nutritionalist, speech and language therapist.
- Supporting your transition from the hospital to short-term convalescence care or placement in longer-term residential care at the earliest opportunity.
